Bed and secured-bed counts are from the Health Facilities Commission's quarterly bed report; the license number, status, expiration, last survey date, original licensure and disciplinary count come from the HFC facility listing pulled for this build.
29 of the 57 ACLFs carry at least one disciplinary action in their HFC listing. Every ACLF links to its own page with the full record, the closest alternatives and the questions worth asking. Tennessee does not post ACLF survey reports in bulk, so the survey itself has to be requested from the facility or through HFC's records portal.
What is the difference between an ACLF and a home for the aged on this list?
A home for the aged offers room, board and personal help to residents who can walk on their own and moves them out when they need health care, while an ACLF can give medications and bring in nursing, therapy and hospice services.
