TL;DR

Roane County (14,126 residents 65+) has two licensed ACLFs in Kingston, nursing homes in Harriman and Rockwood, and Covenant Health Roane. Union County (4,315 residents 65+) has one nursing home and no ACLF or hospital, so families often look to Halls, Powell or Clinton.

Two different rural counties

Roane County, with about 57,395 residents, is built around Kingston, Harriman and Rockwood and the lakes along the Tennessee and Clinch rivers; 24.6% of residents are 65 or older. Union County, with about 21,450 residents, is smaller and more rural, centered on Maynardville and Norris Lake; 20.1% are 65 or older.

Both counties have strong family and church networks, and both have fewer licensed care settings than the region's larger counties.

Roane's two ACLFs are in Kingston and neither reports a secured unit; the county's secured dementia beds are in its two nursing homes. Union County's licensed residential care is a single nursing home, Willow Ridge Center in Maynardville.

Where families look across the line

Union County families usually look south to Halls and Powell in north Knox County, or west to Clinton, all roughly 20 to 30 minutes from Maynardville. Roane County families needing memory care in assisted living often look to Oak Ridge, where four ACLFs report secured beds, or to Farragut and Hardin Valley.

Tennessee licenses are statewide and TennCare CHOICES is a statewide program, so crossing a county line changes nothing about eligibility or the rules that apply.

Making home care work in the country

Home care is often the first choice in rural areas and the hardest to staff. Agencies may require longer minimum visits for outlying addresses or charge travel time. Ask how the agency covers a caregiver's sick day, and keep a backup plan with family or neighbors.

Licensed home health agencies with Roane offices serve the county, and agencies based in Oak Ridge, Clinton and Knoxville serve both counties. ETHRA's AAAD (1-866-836-6678) runs OPTIONS, home-delivered meals and limited transportation in both counties.

Family caregiving carries more of the load

In rural counties, family members often provide most of the care for longer, sometimes for years. That is a strength of these communities and also a risk, because caregivers burn out. Respite matters: TennCare CHOICES members in Groups 2 and 3 can receive up to 216 hours of in-home respite and up to nine days of inpatient respite a year, and ETHRA's caregiver support program can help others.

Adult day health care is another relief valve. At Tennessee's 2025 median it costs about $80 a day, and some programs offer transportation; ask what is available within a reasonable drive.

Planning for the move that may come

Even families committed to care at home benefit from touring one or two assisted living communities in advance. In Roane County that usually means Kingston or Oak Ridge; in Union County, Halls, Powell or Clinton. Knowing the options, prices and waiting lists ahead of time turns a crisis into a decision.

If a nursing home stay becomes necessary, check the CMS record for each home within reach. Roane's two nursing homes sit at very different points on CMS's scale, and Union's single home has its own page with staffing and inspection details.

Local help to call

ETHRA's Area Agency on Aging and Disability serves both counties at 1-866-836-6678 for CHOICES screening, OPTIONS, meals and SHIP Medicare counseling. The East Tennessee District Long-Term Care Ombudsman at ETHRA, 865-691-2551 ext. 4223, advocates for residents of nursing homes and assisted living in both counties.

Transportation

Getting to appointments is a real barrier in rural counties. ETHRA operates public transportation in its region, and some services for older adults include limited rides to medical appointments. Ask the AAAD what is available in your county, and ask any assisted living community in Kingston, Oak Ridge or Halls whether its van covers trips to Knoxville specialists.

Veterans in rural counties

Wartime veterans and surviving spouses in Roane and Union counties should check VA Aid and Attendance, which can add up to $2,424 a month for a single veteran in 2025-26 when care costs reduce countable income. The Senator Ben Atchley State Veterans Home in Knoxville is the nearest state veterans home.

Hospitals

Covenant Health Roane in Harriman is Roane County's hospital on the state's report. Union County has none; residents typically use Tennova North Knoxville in Powell or hospitals in Knoxville. Rockwood and Kingston residents also use Methodist Medical Center in Oak Ridge.
