Remember that the home itself still costs money: mortgage or rent, utilities, food, repairs and property tax. Assisted living rolls most of those into one bill.

When is staying home with help the better choice?

When needs are predictable and fit into scheduled visits, the home is safe or can be made safe, a family member is nearby for the gaps, and the person strongly prefers it.

Home care also works well alongside adult day health care, which costs about $1,733 a month for five days a week at Tennessee's median. TennCare CHOICES and OPTIONS for Community Living can pay for some home services for people who qualify.

When does assisted living make more sense?

When care is needed at unpredictable times, especially at night, when falls are frequent, when isolation is wearing a parent down, or when the family caregiver is near the breaking point.

In rural parts of Union, Roane or eastern Sevier County, finding reliable aides for every shift can be the deciding factor. An ACLF has staff on duty around the clock, which no part-time home care schedule can match.

Does TennCare pay for home care in Knox County?

TennCare CHOICES Groups 2 and 3 can cover personal care visits (up to 2,580 hours a year), adult day care, respite and other home and community services for eligible members, delivered through a managed care plan's care coordinator.

OPTIONS for Community Living offers homemaker, personal care and home-delivered meals with no income limit and a sliding fee, through ETHRA's AAAD at 1-866-836-6678.

How do we decide between the two?

List the hours when help is truly needed, price them at local agency rates, add the home's own costs, and compare the total with written assisted living quotes; then weigh safety, social life and what the person wants.

Tour two or three Knoxville-area communities even if you plan to stay home, so the option is ready if a fall or hospital stay forces a quick decision.
